I've had this happen on multiple occasions with both the black market and the Hydral homeworld remnants.

I had similar issues with outposts. I added a relation to 0013922 from early in the alpha.

Still happening (or happenning again) in v1.009, just had it happen.

Attached a screengrab an new ticket added relationship to this ticket.

Fix suggestion: Make a "PoI/planet list" along the left of the screen (similar to the notifications list on the right) that simply contains every major object in the system. Left click and rt click behavior operates from either the list or from the system view, and rolling over an object in either highlights the object in both (so rolling over something in the list highlights it in the system, and vice versa). Also, setting and/or quick key to show/hide the list.

For 1.010:

* Fixed a bug where the black market's start-of-game seeding logic for "stay at least X pixels away from all planets" was actually only checking the destroyed hydral planet, not the homeworlds.
