I've had this happen as a result of dueling a Burlust warlord as well; it doesn't seem to be exclusive to the very first battle.

First I got "ERROR RESULT" after the first mission. I also got it from sabotaging a quantum power plant.

* Federation points are being removed. Interesting beta-branch experiment, but didn't work out.
