Does it persist if you exit and reload? If so, can you post your world folder?

Yes it does. Where is my world folder?

It should be inside your install folder. Mine is in:

C:\Program Files (x86)\Desura\Common\a-valley-without-wind\RuntimeData\Worlds\

Regrettably, I cannot reproduce the problem. Here's what I tried:

1.1) Loaded Environ A (the save you submitted)
1.2) Found that it was after the completion point of the mission (which I expected, naturally you wouldn't have saved before seeing the bug, but after), so I couldn't test if completion caused rescue in that particular mission.
1.3) Got back to the world map, used a dev-guardian-power to spawn some rescue-survivor missions.
1.4) Successfully completed one of those missions, and it worked just fine (NPC disappeared when they touched the door, and the settlement had 24 residents afterward).
1.5) Found a rescue-mission node in an underground and completed it. I saved before completing and reloaded after the test to make sure it worked properly after save/load and it did. Both tests resulted in a settlement with 25 residents (and the NPC no longer being visible in the mission chunk).

If you can, please:

2.1) Try to find another rescue mission.
2.2) Get the NPC and get most of the way back to the door.
2.3) Quit (it will save).
2.4) Make a copy or backup of your world. Doing this while the world is not loaded is pretty important, as otherwise you generally get a corrupted version because some file was being written to in the middle of the copy.
2.5) Load your world (not the copy).
2.6) Complete the mission.
2.7) If the mission completes successfully (the npc disappears AND your settlement population goes up by 1), then you haven't reproduced the bug either and I guess we can call this one a fluke.
2.8) If the mission fails to complete, or if the npc stays on the screen, or your settlement population does not increase, then you've reproduced the bug: in that case please attach the copy of the world you made in step 2.4 to this mantis record so I can give it a try :)

Thanks!

Just uploaded "Environ A - KDLTest", a copy of the world from just before I completed the underground-rescue-mission in my test. If you want to quickly retest you can load that up and see if it works for you (it worked for me). I wouldn't suggest playing further from it, though, as I made a bit of a mess ;)

All right, I shall play when I have the time. You can put this on hold in the meantime.

Looks like this guy will stay there forever, haha!

Thanks!

Wait -- Keith, if the survivor is there in a former rescue NPC chunk then they should just automatically consider themselves rescued and go to the settlement. That way it's self-healing with something like this.

For 1.054:

* Added some self-correcting checks when loading a chunk such that if it detects a live NPC in a former mission area that was any kind of Rescue-NPC mission, it whisks them away to the settlement. Hopefully we can find the bug that happens occasionally where the NPC is not whisked away on mission-success, but for the time being this will help players who hit that bug.

Thanks :)
